How reflux works

Describe reflux as movement of stomach contents into the oesophagus without assuming every chest or upper-abdominal symptom is reflux.

Opening scene

Reflux describes stomach contents moving back towards the oesophagus. The sensation can be familiar, but the same area of the body can also produce symptoms for other reasons.

Pressure and position

Explore three conceptual scenes. They explain mechanics only and do not predict symptoms.

  • Upright scene: The illustration shows the stomach below the oesophagus in an upright position.
  • Reclined scene: Changing position changes the mechanical relationship, but does not prove a symptom cause.
  • Fuller-stomach scene: A conceptual increase in stomach volume may change pressure; this is not a meal prescription.

Boundary: Chest pain can have causes other than reflux. Seek urgent help for sudden, severe or concerning chest pain.

Make sense of it

The idea in plain language

A ring of muscle and surrounding anatomy help limit backward flow between the stomach and oesophagus. Reflux can occur when that barrier is overcome or relaxes at an unhelpful time. Pressure, position and the contents of the stomach can all be part of the mechanical picture.

This model explains a route, not a personal cause. Burning, regurgitation, cough and upper-abdominal discomfort may need assessment when they persist or change. Chest pain must not be assumed to be reflux: sudden, severe or concerning chest pain needs urgent medical assessment.

Dr Goutham's role is assessment and management, with referral into a specialist anti-reflux or hiatus-hernia surgical pathway when surgery needs consideration. This Hub does not decide whether a medicine, test or procedure is appropriate.

Check the idea

What does the reflux model explain, and what can it not decide?

It explains a possible direction and mechanical context; it cannot identify the cause of an individual's symptoms or rule out another condition.

You can describe the basic reflux route without treating it as a diagnosis.
